Our team, which includes eight social workers and a supervisor, provides mandated case management services to families with a child with a developmental disability. Three revenue enhancement specialists provide support with state and county financial systems. 

The Children’s Developmental Disabilities Unit provides case management services to families who have a child with a developmental disability. We serve children with a wide variety of diagnoses such as Down Syndrome, Autism, genetic disorders, cerebral palsy, and many other diagnoses that may cause developmental/cognitive delays. 

  • Assessments: We meet with families to complete a MnCHOICES assessment to determine eligibility for funding and services, as well as to assist with medical assistance applications. 

  • Services & Support: Once the assessment is complete, we work with the family to determine what available services and supports best meet the child’s needs and assist with putting them in place. We provide support and assistance in helping families navigate a complicated service system. We are available to attend school IEP meetings. We also connect with adaptive recreational programs, medical providers, therapeutic services, Autism programs, advocacy programs, and others to coordinate the child’s benefits.
